Injuries & Incidents

If a player has injured themselves, more then a bump or bruise, whilst playing on the fields it is critical that an injury  form is filled out and lodged. If it is an injury to the head an NRL - COMMUNITY HEAD INJURY RECOGNITION AND REFERRAL FORM will also need to be completed.

INJURIES - If a player is injured please complete the Injury form and provide a copy to the parent, the Secretary of the club and keep  one in the score sheet folder for NSWRL.

FOR HEAD INJURIES - If a player receives a head injury the Medical Trainer must   complete an Injury Report Form and  NRL - COMMUNITY HEAD INJURY RECOGNITION AND REFERRAL FORM & provide a copy to the parent along with a copy of NRL Community Head Injury/Concussion Medical Clearance form. A copy also needs to go to the  Secretary of the club and keep one in the score sheet folder  for NSWRL. A doctor will need to complete the NRL Community Head Injury/Concussion Medical Clearance form before the player can return to playing/training. Please provide 2 copies of your clearance form to our Club Secretary. 
NOTE: If player is not cleared of head injury by a Doctor within 48hrs the player will be excluded from playing/training for 14 days.


INCIDENTS - If an incident occurs either on or off the field at a junior league ground and you think it warrants reporting to the Penrith JRL then please complete the following report and submit to the Club Secretary by 9am Monday following the incident.

Gallagher  Sports   Insurance



If a parent/player would like to make an insurance claim they are required to fill out the following form and return to our Club Secretary who will complete Brumbies club section and email it back to you to lodge with Gallagher Sports Claims.
NOTE: You only have 30 Days from injury date to have your claim lodged with the Insurance.

​Once you have lodged the Insurance Claim form with Gallagher Sports Claims, we encourage you to contact them directly in regards to any claim enquiries:
